MVCC Tuition Fees

A drawing will be held on Jan. 13th, 2025 for one choir member to receive reimbursement of Spring 2025 Tuition.

Entries must fully complete registration by doing the following:

1. Read "Policies and Procedures and Register choir member

2. Read and initial the Acknowledgement of Risks form

3. Pay the tuition in full via Venmo, Cash, or Check

The winner will be notified and reimbursed $55.00 for Spring 2025 Tuition.

Receiving Info from MVCC:

All info regarding rehearsals, performances, and so forth will be sent via email and the MVCC Group Me.

ACKNOWLEDGEMENT OF RISKS AGREEMENT

PLEASE READ CAREFULLY BEFORE INITIALING

I, the undersigned, being at least 18 years of age, hereby represent that I am the parent or legal guardian of the child(ren) named below (individually and collectively referred to hereinafter as "my Child"). This agreement is valid and binding for the entire 2024-25 MVCC season.

I understand that the MVCC will include a variety of activities, including but not limited to singing, art projects, and standing on risers. I also understand that my Child will be performing in front of others.

1. I understand that participating in choir involves RISKS, DANGERS AND HAZARDS, including but not limited to conditions in standing, walking on risers, singing and playing with others. I UNDERSTAND THAT MISHAPS WHICH MAY CAUSE SERIOUS INJURY OR DEATH CAN OCCUR.

2. I HEREBY FULLY RELEASE, INDEMNIFY AND HOLD HARMLESS Morgan Valley Children's Choir, representatives, agents, affiliates, partners, officers, and directors hereinafter collectively (“MVCC”) of and from any and all liability, claims, demands, actions, and causes of action whatsoever for any loss, damage, injury, illness or harm of any kind and nature arising from my Child’s participation in MVCC, INCLUDING CLAIMS OF NEGLIGENCE. In addition, I AGREE TO INDEMNIFY and hold MVCC HARMLESS of and from any harm, injury, damage or loss to persons and/or property which my Child may cause and/or contribute to while participating in the MVCC 2024-25 season

3. I authorize MVCC to call for medical care for my Child or to transport my Child to a hospital if, in the opinion of anyone working at MVCC, medical attention is needed for my Child. I agree that upon the transporting of my Child to any medical facility, clinic or hospital that the responsibility shall be totally fulfilled and MVCC shall not have any further responsibility for my Child.

4. I agree to pay all costs associated with such medical care and related transportation for my Child and INDEMNIFY and hold MVCC HARMLESS from any costs incurred therefore.

5. I warrant and represent that my Child is in good health and there are no special problems associated with the care of my Child and I have left no special instructions regarding my Child, unless contained in WAIVER.

6. By initialing below, I agree to all the Reservation, Payment, No-show, Cancellation and Refund policies, including but not limited to canceling or changing my product(s) before 5 p.m. MST two (2) days prior to the activity date in order to receive a full refund or be assessed the applicable fee; per terms of email or verbal confirmation. I agree to pay MVCC the full cost of all of my Spring 2025 semester $55 by Jan 13, 2025. By doing so I secure my spot in the choir and will also receive a promotional product. Payments made after Jan 13, 2025 may not guarantee a spot in the choir or a choir promotional product. Payments on or after Jan 13, 2025 will require a late fee of $15 per family.

7. To the extent any portion of this Agreement is deemed unenforceable, the remaining portions shall remain in full force and effect. Any legal action against MVCC will be brought in Morgan County, Utah and Utah law will govern.

I have carefully read and I understand this agreement and all of its terms. I understand that this is a RELEASE OF LIABILITY which will legally PREVENT me or any other person from recovering in any lawsuit or in connection with any other legal claim for damages in the event of my Child’s death or any injury to my Child. I also understand that this Agreement and Release is fully binding and applicable for the 2025 season in which the Agreement and Release is signed and is NOT LIMITED in any way to just inherent risks, but covers non-inherent risks as well and injuries sustained by me for any cause or reason without limitation. I nevertheless enter into this Agreement freely and voluntarily and agree that it is binding upon me, my heirs, assigns and legal representatives. I hereby grant exclusive permission to MVCC and its agents to use my child’s name and photograph for the purpose of publicity, public relations, editorial or other advertising purposes without restriction as to frequency or duration. I also agree that neither my minor child nor I will be entitled to any compensation in exchange for my granting this exclusive permission to MVCC and its agents.
